New Patient Information
Important Note: We request that you continue to work in partnership with your primary care physician. Dr. Tetlow is and integrative alternative medicine doctor and unable to act as your primary care physician due to limited office hours and staffing.
Please review the Financial Policy Explanation for more information on why Dr. Tetlow chooses to operate independent of insurance companies.
Payment is due at time of service. At your request, Dr. Tetlow will complete forms for you to submit to your insurance company for reimbursement. We do not process insurance claims. We encourage you to contact your insurance carrier to determine whether holistic, alternative and integrative medicine is covered.
Most lab tests and x-rays that Dr. Tetlow would request are covered by insurance. However, you remain responsible for contacting your carrier to determine what is and is not covered, and if a pre-authorization from your primary care physician is needed.
Dr. Tetlow accepts all major credit cards and personal checks for payment of services.

Your needs are always Dr. Tetlow's number one priority. You can count on her to spend as much time as needed to provide you with the highest level of care.
DEPOSIT & CANCELATION:
You will be required to place a $100 deposit for your appointment via Credit Card when you make the appointment. You will forfeit the deposit if you do not provide a 48 hour (2 days) notice of cancelation. If you have given a 48 hour notice, your deposit will be applied to your balance, or refunded, as applicable.
- New Patient Appointment - 90 Minutes @ $350.00
- Follow Up Appointment - 40 Minutes @ $195.00
- Former Patient (1 year since last visit) - 90 minutes @ $325.00
Discounts available as follows:
Pay for 3 appointments (first appointment and 2 follow ups) in advance and save $90. Pay $650 instead of $740.00.
Pay for 5 appointments (first appointment and 4 follow ups) in advance and save $280. Pay $850 instead of $1,130.00.
The discounted appointments are monthly and the price covers medical management and communication with Dr. Tetlow via phone and email for the applicable months.
Get a FREE follow up appointment (Value of $195), when you refer 3 new patients to Dr. Tetlow and one of them makes and keeps a new patient appointment. Just send us the names, including phone and email addresses and give us permission to contact them regarding our alternative, holistic and integrative medicine services. Please let them know you referred Dr. Tetlow. Send your referrals to: info@beingmybestself.com.
